The following is from the Massachusetts Department of Public Health:
Compliant or C: is when the facility has no findings. They have everything Mass DPH is inspecting against.
Substantially Compliant or SC : is when the facility has minor issues.
Violation or V: is when the facility has serious issues like:
Phantom score less than 4/3/3 (quality control test)
Average dose over 300 mR
No quality assurance program
Using a system not designed for mammography
Only one Interpreting Physician and they are not qualified
Responsible Physician not qualified
Medical Physicist not qualified
Only one Radiological Technologist and not qualified
Last medical physicists report greater than 24 months
No interpretation reports for mammograms
Reports not sent to the Referring physician or patient
No 24 x 30 bucky (missing piece of equipment)
Repeat subcompliant issue from last inspection
